Seven H Care LLC is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our home healthcare team. As an LPN, you will provide skilled nursing care to patients in the comfort of their homes, supporting their health, safety, and quality of life. You will work closely with RNs, caregivers, physicians, and family members to develop and deliver individualized care plans.
π Key Responsibilities
- Deliver direct patient care under the supervision of an RN or physician.
- Monitor vital signs, administer medications, and assist with wound care.
- Observe patient conditions and report changes promptly to supervising clinicians.
- Educate patients and families on treatment plans, medications, and self-care techniques.
- Document all care services provided in accordance with agency and state guidelines.
- Collaborate with the care team to ensure a holistic approach to patient care.
- Maintain patient confidentiality and adhere to HIPAA and ethical standards.
β Qualifications
- Current and valid LPN license in the state of Tennessee.
- Minimum of 1 year of nursing experience, preferably in home health, hospice, or clinical settings.
- CPR Certification (required).
- Strong interpersonal and communication skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and willingness to travel to patient homes.
